PHOENIX – The Arizona Game and Fish Department (AZGFD) reminds hunters that all existing crossbow permits and all Challenged Hunter Access/Mobility Permits (CHAMPs) expired June 30, 2026.

Any person wanting to hunt on or after July 1, 2026 using a crossbow permit (Rule R12-4-216) or CHAMP (Rule R12-4-217) will need to complete a new medical evaluation and submit a new application to AZGFD. Applications are now being accepted in person or by mail at any AZGFD office

You can find the new crossbow permit applications here and the new CHAMP applications here. Please use only these forms. Outdated forms and forms obtained elsewhere will not be accepted.

Please note that Rules R12-4-216 and R12-4-217 define a “Qualified Physician” as a Medical Doctor (M.D.) or a Doctor of Osteopathic Medicine (D.O.). Forms completed by a Nurse Practitioner, Chiropractor or other provider will not be accepted.
